Overweight? Hospitals should turn you away for high blood pressure, diabetes or any other medical issue related to obesity
Cancer or COPD? You smoked. No treatment for you!
Break a limb? No treatment if you did something silly like fall off a ladder.
Driving with no insurance? No treatment.
There are many other situations that could be added.
