Italy got to this point, only so many available respirators, way more people needing them, how do you decide?
You have to rule out severe cases that your experience says probably won't make it anyway
...still not enough respirators, people dying of cancer or heart failure are out
...still not enough, now we're looking at severely mentally retarded
...then we get to where Italy got, choosing younger people and those with kids, the elderly left at home to die
This is harsh, but many areas are going to be making choices as to who dies...it takes courage and resolve to set a policy upfront so the choice isn't made under pressure.

wood, We can make lots more respirators than we are now. The problem is that Trump has not ordered American companies to manufacture them. He is looking for companies to volunteer. He certainly has the power to make super large corporations manufacture protective gear, tests for the virus and respirators. He is in a position to save thousands, maybe hundreds of thousands of lives. He said that we are at war with this disease. If FDR would have asked American companies to volunteer making tanks airplanes and all the other materials for making war, we all would be speaking German now.

I caught something about Tesla engineering a respirator they could make, they were at a stage where they can deliver the first thousand.
It obviously would take companies situated with the right tools and technology. Mask manufacturers should be ramping up on 2/3 shifts until the demand is determined, they know the demand will be there.

Ford is working with 3M and GE to make respirators and ventilators They'll be making masks also.
A respiratory ventilator is not a wind up toy, it is a sophisticated technological machine. Gonna take these manufacturers a little time to get all the construction data squared away and tool up or retool to get these babies rolling.
It is the state governments, i.e., governors, who are responsible for preparing their states for such contingencies. State governments certify hospitals, clinics, and medical facilities within their state, they regulate them, approve expansion of them, authorize upgrades in medical equipment and supplies. State governments license physicians, nurses, and other medical professionals. This is not the responsibility of the federal government.
It is telling that the hardest hit states - New York, California, Washington, New Jersey - are all run by democrats. With over half the cases of COVID-19, NY is the epicenter of this pandemic.
Due to prior viral outbreaks such as the flu and Ebola, governor Cuomo was told that NY needed to beef up the state's defense. He rejected the proposal.
