After the Janet Jackson Super Bowl fiasco, the networks have a delay for this very reason on live broadcasts. This probably happens far more often than you think. I hear news anchors say dumb things all the time, such as the S. D. newscaster saying about the lottery..."You can't play if you don't win!"
Fox in Oakland reported the Korean Airline crash in SF saying "we have now learned the name of the pilot and co-pilot of that aircraft. The pilot was Wee Tu Lo and the co-pilot was Sum Ding Wong." And they say it with a straight face. I'll bet an employee(s) put an intern up to this, writing it on the final copy for broadcast and put up on the teleprompter. Surely someone was fired for this one.
