If you say yes, you're sent to another counter, raise your right hand, say you are who you are, and voila - voter registration card. If you think workers at the DMV are monitoring this when they have lines out the door you're naive.
What's naive is thinking that one doesn't have to fill out voter registration forms at the DMVs, just raise one's right hand.
