It's very simple really. If you do not convert the first time, they chop off your head. People are converting just to save their lives. The fact that you cannot leave once you join is no big deal really. There are a lot of things you can join that you will die if you ever leave.
They have to k**l anyone that leaves because once they join and find out what it really is, they want nothing to do with it, but if they let them leave, there would be a LOT of people leaving it. This is a case where the few suppress the many.
