That's actually a catch 22 problem. The more brains involved, the quicker the problem gets solved. That was the issue with 9/11. Not enough brains were focused on the potential problem and it got out of hand. I understand scale of efficiency, but sometimes it's not worth it. And you mention progressive holdovers----you have to remember some of those folks were Reagan, Clinton, and Bush holdovers, as well.
9/11 has been a potential problem for a long, long time. We discussed it in the 70's - and that was just at th Senior NCO level. There was even a novel or 2 written about that scenario back the day.
