Maybe the artist is trying to imply that Obama is a reptilian descendent of the Neflim, the descendents of the fallen angels that mated with human women and created races of giants way before the time of the flood. In the Bible, Genesis gives 2 paragraphs about that and when you come to it, you say "WTF, where did that come from?", But if you want to know more about that you need to read the book of Enoch (which didn't make it into the Bible) or the Samarian text, or book of Jubilee, all of which give great detail. There is no question that there are pictures in which O's head seems to have scars, but I doubt it would be from "shrinking his head" to look more human. What would you do with the extra brain tissue that you would have to also "shrink"? It would be very strange to see someone with that kind of damage survive the operation.
