The North Korean people are kept in the dark and know little of the rest of the world. The military heads know the score. They know exactly what will happen with the first shell or bomb landing in Seoul or anywhere else.
There is no loyalty to him. There is obedience borne of fear of him because none of them know who can be trusted. For them to take him out would require total cooperation of them all to insure not being ratted out and killed. They cannot trust each other enough to take that chance.
If we took Too Fat out the military would either disintegrate or start.killing each other off in power grabs. Either way, they would not choose suicide.

Worth noting that Kim has already executed 3 of his top 5 generals. That didn't work out well for Stalin.
